Get a copy of your tax transcript. This can be completed online or by mail. Please see this website for full instructions.

Additional Creditors
Our office requires a $31.00 fee to add up to six (6) additional creditors to your current Bankruptcy Case.
- Please download this form, print and completely fill out.
- Include a copy of each creditor statement along with this form.
- Return the completed form, copy of each creditor statement, and your check or money order to our office in person.
If you would like to add more than six (6) creditors to your current Bankruptcy Case, there will be an additional $31.00 fee.
Plan Modification Form
- Please download this form, print and completely fill out.
- Include your two most current pay stubs and any other document(s) from the creditors that you wish to add.
- Include a letter stating any changes to your plan that you are requesting. You must include your signature and the date on that letter.
- Return these documents to our office in person and make an appointment to sign your updated plan.
